Hpv Oral Cancers Slow To Develop But Quick To Spread
HPV-related cancers spread quickly to the lymph nodes. Its not that the tumors spread quickly due to issues with the immune system they spread quickly for unknown reasons. However, they show up first as large swollen lymph nodes because the body has an immune response at the site of the lymph nodes once tumor reaches this area, causing swelling and a noticeable neck mass, Dr. Prendes says.
So, these cancers are often diagnosed when they have already spread to lymph nodes in the neck. However, the good news is due to the excellent response of these HPV-related throat cancers to treatment, even a patient with multiple lymph nodes in the neck containing cancer will still fall into Stage 1 or Stage 2 disease, according to the newest version of the AJCC cancer staging system.
It can take up to 30 years for HPV-related throat cancer to appear, making it most common in adults between the ages of 40 and 60. The Oral Cancer Foundation reports its growing fastest among otherwise healthy men, ages 35 to 55.
Dr. Prendes says researchers have developed a clinical test that searches for the P16 molecular marker in biopsy samples, a marker that indicates HPV-related cancer. Successful treatment and cure rates are high if you know what to look for and know your risk, he says.
Overall, HPV-related throat cancers respond well to surgery and radiation. Between 90 and 95% of diagnosed cases have five-year, disease-free survivorship rates, he says.

Yes, the vaccine can be given to adults between the ages of 27 and 45 who didnt receive all vaccine doses earlier. Adults in this age group benefit less from the vaccine because they are more likely to have been exposed to HPV already. Therefore vaccination is not routinely recommended for people in this age group. How Is Oral Hpv Diagnosed
There is no test that can find the first signs of oral HPV. Your healthcare provider may find oral HPV lesions during routine screenings or exams. But, most of the time, testing only confirms the virus is present in people who already have symptoms.
Your healthcare provider may ask to take a biopsy of any suspicious areas. They can test your tissue sample to see if its cancerous or pre-cancerous. If you do have oropharyngeal cancer, treatment may be more effective if its HPV-positive.

Just as has been seen with cervical HPV infections, only a small number of anal HPV infections progress and become cancerous. What Are The Symptoms Of Hpv
A lump in the neck is the most common and most obvious symptom of HPV-related throat cancer.
In some patients, the cancer can cause ear pain as it presses on nerves that lead to the ear. But, it typically does not cause any pain or difficulty swallowing, even when it becomes large and visible.
“Because the cancer often develops in a crevice, people don’t usually feel it. It can be painless,” says Gross.
He says sometimes the swelling and discomfort are misdiagnosed as an infection. He’s had many patients who have been treated with several rounds of antibiotics, or even had teeth removed, due to a mistaken diagnosis.
“They do well once they get the proper treatment, but it is always frustrating that there has been a delay,” he says.

If you didnt get an HPV vaccine when you were younger, whether because they werent available yet or because you didnt know about it, its not too late. The CDC has changed its guidance to recommend the HPV vaccine for some people up to age 45, especially if you regularly have more than one sexual partner or have left a long-term relationship. The vaccine is not always as effective if youve previously been exposed to HPV. If youre sexually active , you should consider an HPV vaccine.

“Throat cancer is still relatively rare, and though it is not an epidemic, the rate of increase is alarming.” says surgeon Scharukh Jalisi, MD, Chief of Otolaryngology and Medical Director of the Head and Neck Cancer Program at Beth Israel Deaconess Medical Center.
HPV-positive throat cancer is rarely detected early because it advances silently at first.

When HPV infects laryngeal tissues, it can cause excessive expression of HPV E6 and E7 proteins and directly or indirectly affect the stability and activity of proto-oncogenes and tumor suppressor genes. For example, HPV16 E7 protein can activate the cell cycle regulation protein D1 , thereby accelerating the G1-to-S phase in cells and leading to the occurrence and development of laryngeal cancer 40. Zeng et al.41 also supported this view and pointed out the association of positive expression of HPV16 E7 and cyclin D1 with laryngeal cancer clinical staging , with a significant positive correlation between their expression and the incidence of laryngeal cancer. This finding indicates that the occurrence and development of laryngeal cancer is closely related to the overexpression of HPV16 E7 protein and cyclin D1. Therefore, the expression of HPV16 E7 and cyclin D1 must be detected in laryngeal cancer tissues for early diagnosis and early treatment of laryngeal cancer, and both can also be used as biomarkers to determine the malignant degree and prognosis of laryngeal cancer 42.

What Is Oropharyngeal Cancer

For protein level detection, tumor suppressor protein p16 immunohistochemical staining is the main marker to evaluate the status of HPV infection. Positivity for p16 is a good prognostic marker of HPV-associated oropharyngeal squamous cell carcinoma 58, but contrary to the accepted pathogenic and prognostic role of HPV in oropharyngeal malignancies, its role in laryngeal cancer is relatively limited 59. Therefore, this method is not suitable for detecting the infection status of HPV in laryngeal cancer. Also, although IHC can detect virus antigens easily, it has some disadvantages such as false negatives and low sensitivity. Meanwhile, the positive detection rate of IHC may be much lower than that of PCR 60. Thus, IHC is not an ideal testing platform for HPV.
