What Is Perioral Dermatitis
Perioral dermatitis is an inflammatory rash involving the skin around your mouth. The rash may spread up to your nose or even your eyes. In that case, its referred to as periorificial dermatitis.
Perioral dermatitis usually appears as a scaly or red bumpy rash. In darker skin, the lesions may be hyperpigmented or brown.
The bumps may contain fluid, so there may be a clear fluid discharge from the bumps. Slight itching and burning can also occur.
The condition is most common in women between 20 and 45 years old , but it can be seen in all ages, races, and ethnicities. It can occur in children of any age.
Episodes of perioral dermatitis can last weeks and even months. Perioral dermatitis is often chronic and relapsing, but it may resolve after the offending agents are removed.
To Calm The Itching Or Prickly Feeling
- apply something cold, such as a damp cloth or ice pack for up to 20 minutes
- tap or pat the rash instead of scratching it
- do not use perfumed shower gels or creams

How Is Impetigo Treated
Impetigo is typically treated with antibiotics, either as a cream/ointment to put on skin or a medicine taken by mouth:
- When it just affects a small area of the skin , impetigo is treated with antibiotic ointment for 5 days.
- If the infection has spread to other areas of the body or the ointment isn’t working, the doctor may prescribe an antibiotic pill or liquid to be taken for 710 days.
After antibiotic treatment begins, healing should start within a few days. It’s important to make sure that your child takes the medicine as prescribed. Otherwise, a deeper and more serious skin infection could develop.
While the infection is healing, gently wash the skin with clean gauze and antiseptic soap every day. Soak any areas of crusted skin with warm soapy water to help remove the layers of crust .
To keep impetigo from spreading to other parts of the body, the doctor or nurse will probably recommend covering open infected areas with gauze and tape or a loose plastic bandage. Keep your child’s fingernails short and clean to prevent scratching that could lead to a worse infection.
Children can usually return to school or daycare 24 hours after starting treatment.

Can Impetigo Be Prevented
Keeping skin clean can help prevent impetigo. Kids should wash their hands well and often and take baths or showers regularly. Pay special attention to skin injuries , areas of eczema, and rashes such as poison ivy. Keep these areas clean and covered.
Anyone in your family with impetigo should keep their fingernails cut short and the impetigo sores covered with gauze and tape.
To prevent impetigo from spreading among family members, make sure everyone uses their own clothing, sheets, razors, soaps, and towels. Separate the bed linens, towels, and clothing of anyone with impetigo, and wash them in hot water. Keep the surfaces of your kitchen and household clean.
What Can I Expect If Ive Been Diagnosed With Perioral Dermatitis

Expect that youll have to abide by a treatment plan worked out with your healthcare provider. Every treatment plan is different. You might need topical medications or oral medications, or you might simply need to stop doing whats causing the rash . However, most of the time a treatment regimen that effectively controls the perioral dermatitis can be found.
Remember your perioral dermatitis will get worse before it gets better when you stop taking topical steroids. This is normal, and expected.

Clotrimazole Cream Is Often Prescribed
This was the next cream I was prescribed by a different health care practitioner. Clotrimazole is an anti-fungal agent from the azole category of anti-fungals. They have a significant amount of medical research behind them and are generally viewed as safe for external usage.
For me the clotrimazole worked or about a week, but towards the end of the two week treatment period results disappeared and the seborrheic dermatitis quickly came back . At this point, though, I wasnt only fighting the seborrheic dermatitis around the nose, but it had already spread to other portions of my face.
Upon further research it seems that from the different azoles, ketoconazole appears to be the most effective . The ones tested in the linked study were: ketoconazole, bifonazole, miconazole, clotrimazole, flutrimazole, sertaconazole .
Based on this, perhaps the clotrimazole was just a poor choice out of the available options.
Here in Canada, these creams are not available over the counter and a prescription is needed. However, Nizoral is available for purchase in most stores without prescription. My experience with this product was unfavorable as it was far too strong for my skin (more details available here.

Apple Cider Vinegar Is The Most Popular Natural Treatment
This is the most popular natural treatment for seborrheic dermatitis around the nose . However, medical research in this area is very sparse. The only mildly related medical papers I was able to find indicated the use of white vinegar for its acetic acid content. One paper was focused at dogs, while the other is a general overview.
Apple cider vinegar is inexpensive and widely available. When buying an apple cider vinegar it is typically recommended to choose an organic unfiltered apply cider vinegar.
The most popular way to use it, is by mixing it with equal parts water and applying to the seborrheic dermatitis effected area. Once applied, most people recommend leaving it on for ~15 minutes for maximum benefits.
Apple cider was quite effective for me, especially at first, when my seborrheic dermatitis was mainly around the nose. However, it was never able to fully control it or prevent it from spreading. Regardless, many other users praise it for being the ultimate solution.

People with perioral dermatitis have a visible rash around their mouth, says Peter Lio, MD, a dermatologist at Northwestern Memorial Hospital in Chicago.
“Typically, it would present as small red papules, or bumps, around the mouth and sometimes nose and eye areas, he says. Unlike mild irritation in these areas, this rash persists for weeks and beyond.
Additionally, perioral dermatitis is often associated with redness and scaling, says Arielle Nagler, MD, a dermatologist at NYU Langone Health.
Patients often complain of itching and burning, she says. Since certain topicals can exacerbate perioral dermatitis and treatments are all by prescription, medical care is recommended.
A Woman With A Longstanding Rash On The Tip Of Her Nose

A 37-year-old African American woman with a history of biopsy-proven cutaneous sarcoidosis presented with what she described as a rash on her nose. The rash was red, tender, and mild in severity, and it had been present for 7 years. The patient denied any joint aches, recent infections, fevers, chills, and blisters, and she was not on any new medications or personal care products. She also was not on any current treatments for this condition.
Physical examination showed an erythematous plaque and papules on the nasal tip.
What’s your diagnosis?
Based on her history and clinical presentation, the woman received a diagnosis of lupus pernio.
Discussion
Lupus pernio is a cutaneous variant of sarcoidosis, a disease in which abnormal groupings of inflammatory cells can form in multiple organs of the body. While the lungs are the most common site of infection, skin manifestations occur in approximately 25% of patients.1 Sarcoidosis is predisposed in individuals 20 to 40 years of age and in those of Northern European or African American descent.2 However, lupus pernio specifically is much more common among African American women.3,4
While cutaneous sarcoidosis may remit spontaneously without treatment, spontaneous remission of lesions that have been present for more than 2 years is rare. Achieving effective therapy may require prolonged treatment.

How To Treat The Redness
Rosacea redness is treated differently than redness caused by other conditions.
Avoid the ingredients witch hazel and menthol, which can be found in many toners, and other exfoliant products.
Prescription topical ointments can be used to treat the redness. Laser treatment is also an excellent treatment option for persistent rosacea redness on the face.
How Is Perioral Dermatitis Diagnosed

A doctor or dermatologist will often diagnose perioral dermatitis with just a visual examination of your skin, along with your medical history.
The doctor or dermatologist may also perform a skin culture test to rule out a possible infection. During this test, theyll swab a small patch of skin in the affected area. Theyll send the sample to a laboratory to test the skin cells for bacteria or fungi.
They may also perform a skin biopsy, especially if the rash doesnt respond to standard treatments.
There are several common triggers that can result in a perioral dermatitis outbreak. These should be avoided as much as possible.
