Your Healthy Family: Cold or COVID? Doctors advice on what to do

Reviewed on 7/18/2022

The symptoms of ear pain, runny nose, and sore throat can occur with upper respiratory infections including the common cold and viral sore throat as well as sinus infections. Strep throat may also cause similar symptoms. If you have signs of an infection that do not improve, discuss your symptoms with your doctor so that you can receive appropriate treatment.

While the list below can be considered as a guide to educate yourself about these conditions, this is not a substitute for a diagnosis from a health care provider. There are many other medical conditions that also can be associated with your symptoms and signs. Here are a number of those from MedicineNet:

Nhs Expands Covid Symptoms List To Include Runny Nose Sore Throat And Headache

The official NHS website has today been updated with nine additional Covid symptoms, including a runny nose, sore throat and headache.

The NHS website now says symptoms now can include:

  • a high temperature or shivering a high temperature means you feel hot to touch on your chest or back
  • a new, continuous cough this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours
  • a loss or change to your sense of smell or taste
  • shortness of breath
  • a blocked or runny nose
  • loss of appetite
  • diarrhoea
  • feeling sick or being sick

It adds: The symptoms are very similar to symptoms of other illnesses, such as colds and flu.

Previously, it had just emphasised three symptoms a high temperature or chills a new, continuous cough and differences to your sense of smell or taste.

One public health professor, Dr Nisreen Alwan, was among those questioning the timing of the change.

She wrote on Twitter: After two years of shouting from the rooftops to expand the Covid symptom list on the NHS website, it finally happens when people can no longer request a free test!

Allergies Have Chronic Symptoms

COVID-19, like the flu or common cold, is an acute illness, meaning people feel fine until symptoms start showing up.

Allergies, on the other hand, are usually chronic, presenting with symptoms off and on for weeks, months, or even years, Dr. David M. Cutler, family medicine physician at Providence Saint Johns Health Center in Santa Monica, California, told Healthline.

Allergies should not cause a fever or body aches, Arthur said. Generally, no cough unless you have a lot of nasal drainage.

Conversely, itchy eyes and facial pain are more typical of allergies than a COVID-19 infection.

Allergies may also cause wheezing, she said, especially in people with asthma.

Allergy symptoms tend to vary with the environment: worsening with exposure to dust, pollen, or animal dander, whereas cold symptoms tend to persist regardless of time of day, weather, locality, or other environmental factors, Cutler said.

Also, as with COVID-19, colds are more likely to have generalized symptoms like fever, headache, and body aches, whereas allergies usually affect only the respiratory tract, Cutler said.

Allergy symptoms tend to improve with antihistamine and other allergy-specific medication, he said. Colds are more likely to respond to decongestants, acetaminophen, fluids, and rest.

The CDC issued guidance on the differences in symptoms between COVID-19 and seasonal allergies.

Common Colds: Relief For A Stuffy Nose Cough And Sore Throat

There are no treatments that fight cold viruses directly. But nasal sprays and painkillers like ibuprofen and acetaminophen can provide some relief from cold symptoms. Many other treatments have either not been studied well enough or have no proven benefit.

Colds are very common: On average, adults come down with a cold 2 to 4 times a year, and children have as many as 6 to 8 colds a year. The reason that colds are so common is that they can be caused by very many different kinds of viruses. So having had one virus doesn’t make you immune to other cold viruses.

Colds usually go away on their own after about one to two weeks. Although the symptoms such as a runny or stuffy nose, cough and headache can be bothersome, you don’t need to take medication. None of the currently available treatments can shorten the length of a cold. Antibiotics only fight so they dont help in the treatment of simple common colds that are caused by viruses. They can have side effects too, so they should only be used if a bacterial infection develops as a complication of the cold.

Delta Variant Covid Symptoms Include Headaches Sore Throat And Runny Nose

Researchers warn that UKs most widely established variant may be mistaken for milder illness

Headaches, a sore throat and a runny nose are the most common symptoms associated with the UKs most widely established Covid variant, researchers have said.

The data, collected as part of the app-based Zoe Covid symptom study, suggests that the Delta variant first detected in India feels like a bad cold, according to Tim Spector, a professor of genetic epidemiology at Kings College London, who is leading the work.

Covid is acting differently now, its more like a bad cold, he said. People might think theyve just got some sort of seasonal cold, and they still go out to parties we think this is fuelling a lot of the problem. So, whats really important to realise is that since the start of May, weve been looking at the top symptoms in all the app users, and theyre not the same as they were. So, the number one symptom is headache followed by sore throat, runny nose and fever.

According to the NHS, the classic Covid symptoms are fever, cough and loss of smell or taste.

Spector said that with Delta, a cough appears to be the fifth most common symptom, and the loss of smell doesnt make the top 10.

I think the message here is that if youre young and getting milder symptoms any way, it might just feel like a bad cold or some funny feeling do stay at home and do get a test, Spector said.

Despite Symptoms Its Not The Flu

COVID-19 is not the flu.

As one of a class of pathogens known as coronaviruses, COVID-19 is actually more closely related to the common cold than the seasonal flu.

However, despite some overlap, the typical symptoms of COVID-19 are more similar to the flu than the common cold .

The Delta variant, however, may have more cold-like symptoms.

In terms of differentiating between flu and COVID-19, it can be almost impossible to distinguish, Dr. Jake Deutsch, co-founder and clinical director of Cure Urgent Care and Specialty Infusion in New York. Thats why people are recommended to have flu vaccinations so it can at least minimize the risk of flu in light of everything else.

Fevers, body aches, coughing, sneezing could all be equally attributed to them both, so it really means that if theres a concern for flu, theres a concern for COVID-19, Deutsch said.

When and where you get sick might be the best predictor of whether you have a cold, the flu, or COVID-19, Yildirim said.

People living in communities with low vaccination rates and high rates of COVID-19 are more likely to have COVID-19, she said, especially outside of cold and flu season.

However, she said, differentiating becomes more difficult during the winter, when all three diseases may be widespread.

If you have a mild case of COVID-19, the flu, or a cold, treatment is geared toward management of symptoms, said Cutler.

Mild cases of COVID-19 are thought to last approximately 2 weeks, said Cutler.

How To Prevent Catching A Cold

  • staying fit and healthy

How Can I Tell If I Have A Cold Or The Flu

Colds and the flu cause many of the same symptoms. But colds are usually mild, while the flu tends to be more severe.

A cold often starts with feeling tired, sneezing, coughing, and having a runny nose. You may not have a fever, or you may run a low feverâjust 1 or 2 degrees higher than usual. You may have muscle aches, a scratchy or sore throat, watery eyes, and a headache.

The flu starts suddenly and hits hard. You will probably feel weak and tired and have a fever, dry cough, a runny nose, chills, muscle aches, a bad headache, eye pain, and a sore throat. It usually takes longer to get over the flu than a cold.

What Are The Symptoms To Look Out For

The new list of Covid symptoms has been expanded from the original three:

  • a new, continuous cough
  • a fever or high temperature
  • loss of or change to smell or taste

It now includes nine other symptoms which are common signs of other respiratory infections too:

  • shortness of breath
  • feeling tired or exhausted, lack of energy
  • muscle aches or pains
  • headache that lasts longer than usual
  • blocked or runny nose
  • sore throat, stuffy or runny nose
  • loss of appetite
  • diarrhoea
  • feeling sick or being sick

It’s not possible to tell if you have Covid-19, flu or another respiratory infection based on symptoms alone, official advice says.

But spotting these symptoms can help to reduce the spread to other more vulnerable people.

If you have any of these symptoms plus a high temperature or you don’t feel well enough to go to work or school, you’re advised to stay at home.

Common Causes Of Sore Throat

How to treat a cold | NHS
  • Cold and Flu: The common cold and seasonal influenza share many symptoms, including that dreaded sore throat. If youre suffering from a cold or the flu, you may also experience fever, headache, muscle aches, fatigue, cough, runny nose, sneezing, and congestion. Colds are usually milder than flu, and are more likely to include a runny or stuffy nose. Protect yourself with an annual flu vaccine, and know we are here to provide care if you do become ill.
  • COVID-19: Like the common cold and flu, COVID-19 is a viral, respiratory illness that can indeed cause sore throat. However, sore throat doesnt seem to be a particularly common symptom of the novel coronavirus. One study, commissioned by the World Health Organization , found that out of more than 55,000 confirmed cases, only 13.9 percent of people reported a sore throat. Get a COVID-19 test if youve been around someone who tested positive, or are exhibiting other COVID-19 symptoms, such as cough, difficulty breathing, and/or fever, along with chills, muscle pain, headache, and any new loss of taste or smell.
  • Allergies: When your immune system overreacts to a foreign substance , it can trigger an allergic response. Severe reactions are possible, but for common allergies, youll likely experience itchy eyes, sneezing, runny nose, cough, congestion and headache. A post-nasal drip can lead to an allergy-induced sore throat.

    What Tests Will I Need

Runny nose, sore throat and headache should be added as Covid symptoms ...

Most of the time, your healthcare provider will diagnose you based on your symptoms and a physical exam. You may need a throat swab to diagnose strep throat. Your healthcare provider uses a stick with a cotton swab at the end to take a throat sample. They analyze the sample for bacteria to see if you have an infection.

You may need other tests, such as a blood test or imaging scans, to rule out other infections.
